DO YOU HAVE AN INJURY TO REPORT?

Please take the time to read all of this information and follow these instructions to help with your injury.

1. Please inform your coach and get an accident report filled out please see the below form.

2. Please make sure that this accident report is given to our safety director and then please email our Safety Director, Jennifer Clarke  @ jclarke@assistivetechworks.com

3. If you did receive medical treatment for your injury please fill out the insurance form below and have it sent to the safety director to be signed.

4. If your child/player does seek medical treatment for an injury then you will need to have a release form returned to the safety director before the child/player can return to play. Please see the form below.

5. When in doubt of an injury and if it should be report, please take the time to fill out the forms to help.
